The framework was submitted to the American Bar Association, state bar committees, and the Corporate Legal Operations Consortium (CLOC) for consideration as an industry reference standard. Four of those five failure modes produce no warning sign at all -- the output looks right, even when it isn't -- which is why having the right checks in place before problems reach decision-makers matters.
