State disciplines health care providers
OLYMPIA -- The Washington State Department of Health has taken disciplinary actions or withdrawn charges against the following health care providers in our state.
The department’s Health Systems Quality Assurance Division works with boards, commissions, and advisory committees to set licensing standards for more than 80 health care professions (e.g., dentists, nurses, counselors). Information about disciplinary action taken against medical doctors and physician assistants can be found on the Washington Medical Commission (WMC) website. Questions about WMC disciplinary actions can be sent to media@wmc.wa.gov.
Information about health care providers is on the agency website. Click on “Look up a health care provider license” in the “How Do I?” section of the Department of Health website (doh.wa.gov). The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are encouraged to call and report their complaint.
Chelan County
In January 2021 the Nursing Commission charged registered nurse Hope E. Roe (RN00097983) with unprofessional conduct. Roe allegedly didn’t disclose two previous convictions of driving under the influence.
Clark County
In January 2021 the secretary of health ended conditions on the certified nursing assistant credential of Sarah Michelle Hozack (NC10087255).
In December 2020 the Dental Commission ended conditions on the dentist and general anesthesia credentials of Thomas M. Kreger (DE00005279, GA10000108).
In January 2021 the secretary of health ended conditions on the registered nursing assistant credential of Chelsey Rae Atkins (NA60523460).
In January 2021 the secretary of health ended probation on the certified nursing assistant credential of Michelle Shree Gray (NC60356138).
King County
In December 2020 the secretary of health conditionally granted an agency-affiliated counselor credential to Alexander Anthony Coleman (CG61104293) and placed him on probation for at least two years. In 2018 and again in 2019 Coleman was convicted of third-degree theft. In 2019 Coleman was convicted of first-degree criminal trespass.
In January 2021 the Medical Assistant Program charged registered medical assistant and certified medical assistant Christopher Michael Greaves (MR60566374, CM60433332) with unprofessional conduct. Greaves allegedly didn’t comply with a 2019 stipulation requiring him to reimburse the program for $1,000 in costs, as well as to complete continuing education in scope of practice, and in law and ethics.
In January 2021 the secretary of health ended probation on the registered nursing assistant credential of Bailey M. Burton (NA00190780).
In December 2020 the secretary of health conditionally granted a substance use disorder professional trainee credential to Kevin L. Allen (CO61034469) and placed him on probation for at least three years. Allen had 20 criminal convictions between 1989 and 2018, including theft, drug convictions and criminal trespassing.
In January 2021 the secretary of health ended conditions on the registered nursing assistant credential of Rusiate Natekuru (NA60026572).
Mason County
In January 2021 the Nursing Commission charged registered nurse Lindsey Jane Rooks (RN00154100) with unprofessional conduct. In 2018 Rooks was convicted of driving under the influence – heroin.
Pierce County
In December 2020 the Dental Commission ended conditions on the dental assistant credential of Barbara Jean Eckiss (D160804271).
In January 2021 the Certified Behavioral Technician Program charged certified behavioral technician Serenity Devon Isom (CB61000516) with unprofessional conduct. In December 2019, the Washington State Department of Social and Health Services (DSHS) determined Isom neglected a vulnerable adult. Isom was placed on the DSHS long-term abuse and neglect registry in April 2020, and is prohibited from being employed in the care of or having unsupervised access to vulnerable adults.
In January 2021 the secretary of health conditionally granted a substance use disorder professional credential to Bruce Wayne Davis (CP61097177). Davis must continue to comply with probationary conditions placed on his substance use disorder professional trainee credential in 2020.
In January 2021 the X-Ray Technician Program and the Medical Assistant Program charged X-ray technician and medical assistant Lucas Earl Underbrink (CM60854092, XT60884453) with unprofessional conduct. Underbrink was convicted in June 2020 of second-degree assault, domestic violence and third-degree rape-domestic violence.
Snohomish County
In December 2020 the Nursing Assistant Program ended conditions on the registered nursing assistant credential of Svetlana Valentinovna Karpenko (NA00114042).
In December 2020 the Nursing Commission withdrew charges against licensed practical nurse Elizabeth Cerdena Clausen (LP00039972).
Spokane County
In January 2021 the secretary of health conditionally granted a substance use disorder professional credential to Andrew Phillip Cotter (CP61105599). Cotter must abide by conditions imposed when he received a substance use disorder professional trainee credential in 2019.
In January 2021 the Medical Assistant Program charged medical assistant Sarah Eilene Smith (CM60376576) with unprofessional conduct for allegedly being under the influence of a controlled substance at work.
Out of State
California: In January 2021 the Massage Therapist Program charged massage therapist Danbo Zhang (MA60798158) with unprofessional conduct for alleged misrepresentation or concealment of a material fact in obtaining a license.
Oregon: In January 2021 the secretary of health ended probation on the certified nursing assistant credential of Nadezhda Aleksandrovna Dron (NC60703108).
Note to Editors: Health care providers charged with unprofessional conduct have 20 days to respond to the Department of Health in writing. The case then enters the settlement process. If no disciplinary agreement can be reached, the case will go to a hearing.
