State disciplines health care providers
OLYMPIA -- The Washington State Department of Health has taken disciplinary actions or withdrawn charges against health care providers in our state.
The department’s Health Systems Quality Assurance Division works with boards, commissions, and advisory committees to set licensing standards for more than 80 health care professions (e.g., dentists, nurses, counselors). Information about disciplinary action taken against medical doctors and physician assistants can be found on the Washington Medical Commission (WMC) website. Questions about WMC disciplinary actions can be sent to media@wmc.wa.gov.
Information about health care providers is on the agency website. Click on “Look up a health care provider license” in the “How Do I?” section of the Department of Health website (doh.wa.gov). The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are encouraged to call and report their complaint.
Asotin County
In January 2020 the Nursing Assistant Program charged certified nursing assistant John J. Graves (NC10043616) with unprofessional conduct. In 2019 Graves was convicted of two counts of third-degree malicious mischief-domestic violence, and of one count each of disorderly conduct, resisting arrest, third-degree assault-domestic violence, unlawful imprisonment-domestic violence, and second-degree unlawful possession of a firearm.
Clark County
In January 2020 the Dental Commission filed an amended statement of charges against dentist and moderate sedation with parenteral agents permit holder Ellis Burke Jardine (DE60339865, CS60495559) to add legal references.
King County
In January 2020 the secretary of health conditionally granted a massage therapist credential to Benjamin Ray Gallagher (MA60943475) and placed him on probation for at least two years. On his application, Gallagher admitted attempting to shoplift.
In January 2020 the Nursing Assistant Program entered an agreement with certified nursing assistant Becaley Yolanda Elsie (NC10049409) that lifts the suspension of her credential. Elsie must complete continuing education in restraints and restraint use, managing behaviors, and law and ethics. Elsie’s license was suspended in 2019 after the state Department of Social and Health Services determined that she abused a vulnerable adult.
In January 2020 the Massage Therapist Program charged massage therapist Dena Jameela (MA00013484) with unprofessional conduct. Jameela allegedly didn’t supply proof of having completed required continuing education.
Pacific County
In January 2020 the secretary of health ended conditions on the paramedic credential of Raela D. Wetterauer, also known as Raela D. McVey (ES00117356).
Snohomish County
In January 2020 the Nursing Assistant Program entered an agreement with certified nursing assistant Khaddy F. Janneh (NC60143356) that places her on probation for at least one year, fines her $250, and requires her to complete continuing education in law and ethics. The nursing assistant allegedly used both true and false information to get registered and certified nursing assistant credentials, and used two identities to get medical assistance benefits for which she was ineligible. The program also revoked the registered nursing assistant credential Janneh received under the name Khaddy Fatajo (NA00142366).
In January 2020 the secretary of health conditionally granted an agency-affiliated counselor credential to Travis Lee Boston (CG60997048) and placed him on probation for at least two years. In 2015 Boston was convicted of first-degree possession of stolen property.
In January 2020 the Massage Therapist Program charged massage therapist Charlene Kimball, also known as Charlene Van Alstine (MA00009115), with unprofessional conduct. Kimball allegedly didn’t complete required continuing education.
Thurston County
In January 2020 the Nursing Assistant Program entered an agreement with certified nursing assistant Chad Hugh Smith (NC60744044) that requires him to participate in a substance abuse monitoring program. Smith reported to work and cared for residents at a memory care facility while under the influence of alcohol.
In January 2020 the Nursing Assistant Program withdrew a statement of charges against registered nursing assistant Kristopher Andrew Fehlig (NA00178499).
Indiana: In January 2020 the secretary of health ended conditions on the mental health counselor and substance use disorder professional credentials of Robert Douglas Baize (LH00005969, CP00005724).
Oregon: In January 2020 the Massage Therapist Program charged massage therapist Josandra McLellan (MA00007804) with unprofessional conduct. McLellan allegedly didn’t supply proof of having completed required continuing education.
Note to Editors: Health care providers charged with unprofessional conduct have 20 days to respond to the Department of Health in writing. The case then enters the settlement process. If no disciplinary agreement can be reached, the case will go to a hearing.
