State revokes, suspends licenses, certifications, registrations of health care providers
OLYMPIA -- The Washington State Department of Health has revoked or suspended the licenses, certifications, or registrations of health care providers in our state. The department has also immediately suspended the credentials of people who have been prohibited from practicing in other states.
The department’s Health Systems Quality Assurance Division works with boards, commissions and advisory committees to set licensing standards for more than 80 health care professions (e.g., dentists, nurses, counselors). Information about disciplinary action taken against medical doctors and physician assistants can be found on the Washington Medical Commission (WMC) website. Questions about WMC disciplinary actions can be sent to media@wmc.wa.gov.
Information about health care providers is on the agency’s website. Click on “Look up a health care provider license” in the “How Do I?” section of the Department of Health home page (doh.wa.gov). The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.
Benton County
In February 2020 the secretary of health revoked the home care aide credential of Rebecca Elizabeth Coronado (HM60356350). Coronado admitted that she took narcotics for her own use while she worked for an assisted living facility. The state Department of Social and Health Services disqualified Coronado from caring for vulnerable adults, juveniles, and children. That prohibits her from home care aide certification.
Cowlitz County
In February 2020 the Nursing Assistant Program charged registered nursing assistant Morgan Christine Lewellen (NA60249249) with unprofessional conduct and immediately suspended her credential. Lewellen took a client’s electronic benefits transfer card without the client’s permission. Lewellen and her mother used the card to spend $162 on items that weren’t for the client’s use or benefit. Lewellen lied to law enforcement about having permission to use the card, and about having given the card to anyone else. In 2020 Lewellen was convicted of two counts of third-degree theft, and one count of making a false or misleading statement to a public servant. The state Department of Social and Health Services placed Lewellen on its long-term care abuse and neglect registry after determining that she financially exploited a vulnerable adult. That prohibits Lewellen from being employed in caring for or having unsupervised access to vulnerable adults.
King County
In February 2020 the Nursing Assistant Program indefinitely suspended the certified nursing assistant credential of Ronithe Lineus (NC60783937). After determining that Lineus financially exploited a vulnerable adult, the state Department of Social and Health Services placed her on its long-term care abuse and neglect registry. That prevents her from being employed in caring for or having unsupervised access to vulnerable adults.
In February 2020 the Agency-Affiliated Counselor Program entered an agreement with agency-affiliated counselor Maureen Tsetska Gray (CG60798958) that indefinitely suspends her credential. Gray had a personal and sexual relationship with, and married, a former client.
In February 2020 the Nursing Assistant Program charged registered nursing assistant Jacqueline R. Ramos (NA00099090) with unprofessional conduct and immediately suspended her credential. While working at an adult family home, the nursing assistant continued to sign out for medications a resident ran out of and didn’t receive. After determining that Ramos neglected a vulnerable adult, the state Department of Social and Health Services placed her on its long-term care abuse and neglect registry. That bars her from being employed in caring for or having unsupervised access to vulnerable adults.
Pierce County
In February 2020 the Nursing Commission entered an agreement with licensed practical nurse credential of Stephanie Renae Jackson, also known as Stephanie Evans (LP00050713), that indefinitely suspends her credential. Jackson didn’t complete a required refresher course.
In February 2020 the secretary of health suspended for at least 10 years the registered nursing assistant credential of Bovae Leonjray Strickland, also known as Rovae L. Strickland (NA60757604). In 2019 Strickland was convicted of second-degree burglary with allegation of domestic violence, third-degree assault, and tampering with a witness with allegation of domestic violence.
In February 2020 the secretary of health suspended for at least three years the certified nursing assistant credential of John Goon (NC60684121). When Goon visited a park to meet a minor he had been texting, he was confronted by an adult male Goon hit with his car as Goon tried to leave the scene. In 2019 Goon was charged with second-degree assault.
Spokane County
In February 2020 the Pharmacy Commission indefinitely suspended the pharmacy technician and pharmacy assistant credentials of Eric Todd McReynolds (VA60756393, VB60687743), who admitted diverting controlled substances from his employer.
In February 2020 the Nursing Assistant Program charged registered nursing assistant Haylee Rene Delzer (NA60784791) with unprofessional conduct and immediately suspended her credential. The state Department of Social and Health Services determined that Delzer abandoned a vulnerable adult, and placed her on its long-term care abuse and neglect registry. She can’t be employed in caring for or having unsupervised access to vulnerable adults.
Stevens County
In February 2020 the Nursing Assistant Program charged registered nursing assistant and certified nursing assistant Kara Catherine Frederick (NA60752498, NC60828717) with unprofessional conduct and immediately suspended her credentials. The state Department of Social and Health Services determined that Frederick abused a vulnerable adult. Her placement on its long-term care abuse and neglect registry prohibits her from being employed in caring for or having unsupervised access to vulnerable adults.
Thurston County
In February 2020 the Nursing Assistant Program charged certified nursing assistant Donnell Renee Drake (NC60588200) with unprofessional conduct and immediately suspended her credential. While working as a caregiver, Drake opened credit card accounts using a client’s identity, and used the accounts to make charges that didn’t benefit the client. The state Department of Social and Health Services, which determined that Drake financially exploited a vulnerable adult, placed her on its long-term care abuse and neglect registry. That renders Drake unable to be employed in caring for or having unsupervised access to vulnerable adults.
In February 2020 the secretary of health indefinitely suspended the registered nursing assistant credential of Julie Shipe (NA60545635), who didn’t respond by the deadline to an inquiry about a report of unprofessional conduct related to financial exploitation of clients.
Whatcom County
In February 2020 the Veterinary Board indefinitely suspended the veterinarian credential of Erin Vonderahe-Hargrove (VT60123457), who didn’t complete required continuing education.
Louisiana: In February 2020 the Nursing Commission entered an agreement with registered nurse Heidi Hartenstein Benoit (RN60635790) that indefinitely suspends her credential. In 2019 Benoit surrendered her Louisiana nurse license.
Michigan: In February 2020 the Dental Commission indefinitely suspended the dentist credential of Bill Kawa (DE00007308), who didn’t make an appointment for a required examination.
Oregon: In February 2020 the Substance Use Disorder Professional Program indefinitely suspended the substance use disorder professional credential of Nicole Rayann Hamberger (CO60856270), who didn’t comply with a substance abuse monitoring contract.
