State revokes, suspends licenses, certifications, registrations of health care providers

OLYMPIA -- The Washington State Department of Health has revoked or suspended the licenses, certifications, or registrations of the following health care providers in our state. The department has also immediately suspended the credentials of people who have been prohibited from practicing in other states.

The department’s Health Systems Quality Assurance Division works with boards, commissions and advisory committees to set licensing standards for more than 80 health care professions (e.g., dentists, nurses, counselors). Information about disciplinary action taken against medical doctors and physician assistants can be found on the Washington Medical Commission (WMC) website. Questions about WMC disciplinary actions can be sent to media@wmc.wa.gov.

Information about health care providers is on the agency’s website. Click on “Look up a health care provider license” in the “How Do I?” section of the Department of Health home page (doh.wa.gov). The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.

Clallam County

In December 2020 the secretary of health suspended for at least five years the certified nursing assistant and registered nursing assistant credentials of Jessica Renee Nikkel (NC60921393, NA60890704). Nikkel took two debit cards and a credit card without authorization from patients at her place of her employment and used them to purchase several items for herself. Nikkel was charged with first-degree identity theft and second-degree possession of stolen property – access device. The Washington State Department of Social and Health Services substantiated allegations that Mikkel financially exploited a vulnerable adult.

Douglas County

In December 2020 the Nursing Assistant Program indefinitely suspended the certified nursing assistant credential of Stephanie Lynn Nelson (NC60325356), who didn’t abide by a substance abuse monitoring contract.

King County

In December 2020 the Agency-Affiliated Counselor Program indefinitely suspended the agency-affiliated counselor credential of Paul Weaver Campiche (CG60841991), who didn’t fulfill a requirement to submit performance evaluations or unemployment declarations.

Klickitat County

In December 2020 the Nursing Assistant Program entered an agreement with certified nursing assistant Max Daniel Long (NC60918768) that suspends his credential for at least 10 years. In 2020 Long was convicted of third-degree child rape.

Lincoln County

In December 2020 the Nursing Commission indefinitely suspended the registered nurse credential of Elizabeth Anne Frost (RN60483265). In August 2020 Frost was convicted in Arizona of driving or actual physical control while under the extreme influence of intoxicating liquor.

Mason County

In December 2020 the Nursing Assistant Program entered an agreement with registered nursing assistant Adriane A. Eichhorn (NA60794804) that indefinitely suspends Eichhorn’s credential. Eichhorn didn’t fulfill a requirement to enter a substance abuse monitoring program.

In December 2020 the Dental Commission entered an agreement with dental assistant Shannon Marie Woodhead (D160037201) under which she surrendered her credential. Woodhead didn’t comply with a requirement to enter a substance abuse monitoring program.

Pierce County

In December 2020 the Agency-Affiliated Counselor Program entered an agreement with agency-affiliated counselor Joshua Quaintance (CG60855518) that indefinitely suspends his credential. In 2020 Quaintance was charged with three counts of first-degree possession of depictions of a minor engaged in sexually explicit conduct, and with two counts of second-degree possession of depictions of a minor engaged in sexually explicit conduct.

In December 2020 the Nursing Commission indefinitely suspended the registered nurse credential of Thomas L. Stryker (RN60085678), who didn’t comply with a substance abuse monitoring contract.

Snohomish County

In December 2020 the Massage Therapist Program entered an agreement with massage therapist Xiaohua Jiang (MA60637001) that indefinitely suspends Jiang’s credential. Jiang didn’t respond to a Department of Health inquiry about allegations that she had been arrested and charged with money laundering, and with promotion of prostitution. She didn’t furnish information the department requested.

Spokane County

In December 2020 the Pharmacy Commission suspended for at least two years the pharmacy technician credential of Lotta M. Gaab (VA60253937). In 2019 Gaab was charged in Idaho with unlawful possession of controlled substance. Gaab, who didn’t appear as scheduled in court, didn’t respond to an investigator’s inquiry.

In December 2020 the secretary of health indefinitely suspended the substance use disorder professional trainee, substance use disorder professional, and agency-affiliated counselor credentials of Cory L. Bromley (CO60632489, CP60835848, CG60723761). Bromley didn’t comply with a requirement to enter a substance abuse monitoring program.

Thurston County

In December 2020 the Nursing Commission indefinitely suspended the registered nurse credential of Marcella Lynn Ramos (RN60298763), who didn’t comply with a substance abuse monitoring contract.

Whitman County

In December 2020 the Pharmacy Commission denied the application of pharmacy technician Courtney Leora Fielder (VA60540119) to reactivate her credential and indefinitely suspended her credential. In 2019 Fielder received deferred prosecution on charges of driving under the influence, and of third-degree abandonment of a dependent person.

Out of State

Idaho: In December 2020 the Nursing Commission indefinitely suspended the registered nurse credential of Veronica Kay Franco (RN60756951), who left her hospital nursing assignment without transferring responsibilities.
