State revokes, suspends licenses, certifications, registrations of health care providers

OLYMPIA -- The Washington State Department of Health has revoked or suspended the licenses, certifications, or registrations of health care providers in our state. The department has also immediately suspended the credentials of people who have been prohibited from practicing in other states.

The department’s Health Systems Quality Assurance Division works with boards, commissions and advisory committees to set licensing standards for more than 80 health care professions (e.g., dentists, nurses, counselors). Information about disciplinary action taken against medical doctors and physician assistants can be found on the Washington Medical Commission (WMC) website. Information about health care providers is on the agency’s website. Click on “Look up a health care provider license” in the “How Do I?” section of the Department of Health home page (doh.wa.gov). The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.

King County

In March 2020 the Dental Commission entered an agreement with dentist Charles Henry Pan (DE00007305) under which he surrendered his credential. Pan’s license was suspended in 2019 in connection with deficient infection control practices, recordkeeping, and patient treatment.

Kitsap County

In March 2020 the secretary of health revoked the certified nursing assistant credential of Meaghanne Ann Marzolf (NC60688884). In 2019 Marzolf was convicted of first-degree attempted child rape, commercial sexual abuse of a minor, and second-degree promoting prostitution.

Lewis County

In March 2020 the secretary of health revoked the home care aide credential of Toni Nichole Warden (HM60826852). In 2019 the state Department of Social and Health Services disqualified Warden from caring for vulnerable people, thus prohibiting Warden from home care aide certification.

Spokane County

In March 2020 the Nursing Assistant Program suspended for at least three years the registered nursing assistant credential of Brandi Marie-Ann Farnworth (NA60676120). In 2019 Farnworth was convicted of second-degree assault.

In March 2020 the Nursing Commission entered an agreement with registered nurse Diana M. Sage (RN00137344) that indefinitely suspends her credential. Sage declined to participate in a required substance abuse monitoring program.

In March 2020 the Nursing Assistant Program suspended for at least one year the registered nursing assistant credential of Robert Quinn Blockton (NA60780967). The state Department of Social and Health Services determined that Blockton neglected a vulnerable adult, and placed him on its long-term care abuse and neglect registry. That bars him from being employed in caring for or having unsupervised access to vulnerable adults.

Thurston County

In February 2020 the Veterinary Board indefinitely suspended the veterinary technician credential of Crystal Cherie Pogue-Leyva (AT60440367), who didn’t fulfill a requirement to provide proof of having completed missing continuing education hours.

Out of State

Minnesota: In March 2020 the Nursing Commission indefinitely suspended the registered nurse credential of Zakaria Muigai Chege (RN60886364) with unprofessional conduct. When Chege applied for a Washington nurse license in 2018, he attested that he’d never been convicted of a crime. In 2009 and again in 2012 Chege was convicted in Minnesota of driving while intoxicated – operate motor vehicle under influence of alcohol. In 2017 Chege was convicted in Minnesota of financial transaction card fraud-use-no consent. Chege’s Minnesota registered nurse license was revoked in 2018. His California registered nurse license was revoked in 2019.

Oregon: In March 2020 the secretary of health indefinitely suspended the massage therapist credential of Josandra McLellan (MA00007804), who didn’t supply proof of having completed required continuing education.
