State revokes, suspends licenses, certifications, registrations of health care providers
OLYMPIA -- The Washington State Department of Health has revoked or suspended the licenses, certifications, or registrations of health care providers in our state. The department has also immediately suspended the credentials of people who have been prohibited from practicing in other states.
The department’s Health Systems Quality Assurance Division works with boards, commissions and advisory committees to set licensing standards for more than 80 health care professions (e.g., dentists, nurses, counselors). Information about disciplinary action taken against medical doctors and physician assistants can be found on the Washington Medical Commission (WMC) website. Questions about WMC disciplinary actions can be sent to media@wmc.wa.gov.
Information about health care providers is on the agency’s website. Click on “Look up a health care provider license” in the “How Do I?” section of the Department of Health home page (doh.wa.gov). The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.
Benton County
In May 2020 the secretary of health indefinitely suspended the registered nursing assistant and agency-affiliated counselor credentials of Raeshel D. Lawrence (NA60745853, CG60854777). Lawrence, who made a false report to police of a home invasion, was involved in creating an online campaign that raised about $2,500 for herself. In 2019 she was charged with second-degree theft, and with false reporting.
Island County
In April 2020 the secretary of health suspended for at least three years the certified nursing assistant credential of Terrance Omarh Gibbs (NC60070547). Gibbs borrowed a car from a resident at a facility where he worked, stole about $900 in cash, and repaid $300. Gibbs didn’t respond to a Department of Health inquiry.
King County
In May 2020 the secretary of health suspended for at least five years the massage therapist credential of Shawn C. Husmann (MA60374127). In 2019 Husmann was charged with indecent liberties in connection with improperly touching patients.
Kitsap County
In April 2020 the secretary of health revoked the home care aide credential of Tyler Freeman Williams Jr. (HM60541369). The Washington State Department of Social and Health Services, which found that Williams neglected a vulnerable adult, disqualified Williams from caring for vulnerable adults, juveniles, and children. That bars Williams from home care aide certification.
Pierce County
In April 2020 the secretary of health indefinitely suspended the registered nursing assistant credential of Cassondra N. Lynch (NA00187166). While Lynch was providing home care to a patient, Lynch’s supervisor found her with unopened beer cans and an empty beer can. Lynch took a breathalyzer test and was immediately terminated. She declined to be driven back to the agency’s office and drove away in her own vehicle.
Snohomish County
In April 2020 the Veterinary Board indefinitely suspended the veterinary technician credential of Jessica Rosemarie Brunsdon (AT60530401), who didn’t complete required continuing education, but attested that she had.
In May 2020 the secretary of health indefinitely suspended the massage therapist credential of Xiaohua Jiang (MA60637001). Jiang didn’t respond to a Department of Health inquiry about allegations that she had been arrested and charged with money laundering, and with promotion of prostitution. She didn’t furnish information the department requested.
Spokane County
In April 2020 the secretary of health indefinitely suspended the registered nursing assistant credential of Anteneh T. Taye (NA60973824). Taye, who reported for work while under the influence of alcohol, didn’t respond to a Department of Health inquiry.
In April 2020 the secretary of health indefinitely suspended the massage therapist credential of Dan Guo (MA60322250). Guo owned a business where an unlicensed person offered to provide a massage. Investigators discovered several rule violations at the facility.
Michigan: In April 2020 the Massage Program entered an agreement with massage therapist Hong Juan Zhang (MA60209722) that suspends his credential for at least one year. In 2019 Zhang was convicted of two counts of permitting unlicensed massage therapy.
Texas: In April 2020 the Nursing Commission charged registered nurse and advanced registered nurse practitioner Sara Jane Makielski (RN60804173, AP60804183) with unprofessional conduct and immediately suspended her credentials. In 2019 Makielski surrendered her Texas registered nurse and advanced practice registered nurse credentials in connection with failing to comply with an order regarding participation in a peer disciplinary program.
