State disciplines health care providers
OLYMPIA -- The Washington State Department of Health has taken disciplinary actions or withdrawn charges against the following health care providers in our state.
The department’s Health Systems Quality Assurance Division works with boards, commissions, and advisory committees to set licensing standards for more than 80 health care professions (e.g., dentists, nurses, counselors). Information about disciplinary action taken against medical doctors and physician assistants can be found on the Washington Medical Commission (WMC) website. Questions about WMC disciplinary actions can be sent to media@wmc.wa.gov.
Information about health care providers is on the agency website. Click on “Look up a health care provider license” in the “How Do I?” section of the Department of Health website (doh.wa.gov). The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are encouraged to call and report their complaint.
Island County
In May 2020 the secretary of health denied an agency-affiliated counselor credential to Jules Marie Minne (CG60981622). In 2019 Minne was convicted of fourth-degree assault – domestic violence. Minne described a history of opiate abuse.
King County
In June 2020 the Nursing Commission charged registered nurse Karen Lynn Reed (RN00100301) with unprofessional conduct. The nurse allegedly didn’t comply with a substance abuse monitoring contract.
In June 2020 the Physical Therapy Board charged physical therapist Judy Galambos (PT00009661) with unprofessional conduct. The physical therapist allegedly made about $6,000 in fraudulent transfers from a client’s bank account, not for the client’s benefit.
In June 2020 the Marriage and Family Therapist Program entered an agreement with marriage and family therapist Michael Wayne Baker (LF60710468) that requires him to undergo evaluation for a substance abuse monitoring program. Should Baker not be required to participate in the program, he’ll be placed on probation for at least two years and fined $1,000. Baker diverted and used nitrous oxide, a legend drug.
In June 2020 the secretary of health withdrew a statement of charges against certified nursing assistant, medical assistant, and emergency medical technician Erin Huehnergarth (NC10084835, CM60426487, ES60336639).
In June 2020 the secretary of health withdrew a notice of intent to issue a cease-and-desist order to Evelyn Louise Wambui Njuki, also known as Everlyn Louise Lopez and as Everlyn Louise Wambui Njuki.
Pierce County
In June 2020 the secretary of health denied a home care aide credential to Brandy Ann Moore (HM60970773). The Washington State Department of Social and Health Services, which determined that Moore more likely than not financially exploited a vulnerable adult, placed Moore on its long-term care abuse and neglect registry. That disqualifies Moore from being employed in caring for or having unsupervised access to vulnerable adults, which in turn prohibits Moore from home care aide certification.
Snohomish County
In June 2020 the Pharmacy Commission charged pharmacist Grant Joseph Walker (PH60168992) with unprofessional conduct. Walker allegedly dispensed medication bearing a label containing incorrect instructions, resulting in discomfort to a patient.
In June 2020 the Pharmacy Commission ended conditions on the pharmacy technician credential of Tricia A. Sherman (VA00053812).
In June 2020 the Nursing Commission charged licensed practical nurse Caroline Diane Moyer (LP00043748) with unprofessional conduct. Moyer allegedly didn’t comply with a substance abuse monitoring program.
Spokane County
In June 2020 the Unlicensed Practice Program notified Melissa Sundby of its intent to issue a cease-and-desist order. Sundby allegedly has no dispensing optician license, but performed activities within a dispensing optician’s scope of practice.
In June 2020 the Agency-Affiliated Counselor Program charged agency-affiliated counselor Marsha Rose Valenzuela (CG60600683) with unprofessional conduct. The counselor allegedly borrowed $150 from a client and didn’t repay it within about a year.
Whatcom County
In June 2020 the Pharmacy Commission charged pharmacy technician Adrian C. Bergstrom (VA00070123) with unprofessional conduct. In 2019 the pharmacy technician pleaded guilty to a federal charge of conspiracy to distribute controlled substances. Bergstrom agreed to receive two packages from China containing fentanyl in exchange for $1,000 to $2,000. He received one package and took it to an associate. Law enforcement intercepted a second package. Bergstrom received no payment.
Yakima County
In June 2020 the Nursing Assistant Program charged certified nursing assistant Sonya Rebecca Elizondo (NC10068870) with unprofessional conduct. Charges say the nursing assistant didn’t respond to a Department of Health inquiry about a complaint alleging she financially exploited a client.
Out of State
California: In June 2020 the Nursing Commission charged registered nurse Ray Lincoln Smith (RN60799547) with unprofessional conduct. In 2020 Smith surrendered his California nurse license after admitting that he mixed two vaccines together and administered the vaccine to patients.
Idaho: In June 2020 the secretary of health ended probation for associate independent clinical social worker, independent clinical social worker, and substance use disorder professional Kirk Ryan Stadelmeyer (SC60202267, LW60903087, CP60081245).
Ohio: In May 2020 the Nursing Commission denied a registered nurse credential to Christina Jean Skinner (RN61064697). In 2020 the Ohio Board of Nursing reprimanded Skinner in connection with medication administration and diversion issues.
Oregon: In June 2020 the Nursing Commission conditionally granted a registered nurse credential to Donald Byron Shumate (RN61061477) and ordered him to undergo evaluation for a substance abuse monitoring program. Shumate must comply with probationary terms on his Oregon nurse license, as well as terms of an Oregon court order. In 1996 Shumate was convicted in Colorado of driving while ability impaired. In 2020 Shumate was convicted in Oregon of unlawful possession of cocaine.
Note to Editors: Health care providers charged with unprofessional conduct have 20 days to respond to the Department of Health in writing. The case then enters the settlement process. If no disciplinary agreement can be reached, the case will go to a hearing.
