State disciplines health care providers            

OLYMPIA -- The Washington State Department of Health has taken disciplinary actions or withdrawn charges against the following health care providers in our state.

The department’s Health Systems Quality Assurance Division works with boards, commissions, and advisory committees to set licensing standards for more than 80 health care professions (e.g., dentists, nurses, counselors). Information about disciplinary action taken against medical doctors and physician assistants can be found on the Washington Medical Commission (WMC) website. Questions about WMC disciplinary actions can be sent to media@wmc.wa.gov.

Information about health care providers is on the agency website. Click on “Look up a health care provider license” in the “How Do I?” section of the Department of Health website (doh.wa.gov). The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are encouraged to call and report their complaint.

Benton County

In December 2020 the Nursing Commission ended probation for registered nurse Rebecca Anne Groom (RN00166087).

Clark County

In December 2020 the Nursing Commission ended probation for licensed practical nurse Wai-Ming Mak (LP60171436).

Franklin County

In December 2020 the Nursing Assistant Program filed an amended statement of charges against registered nursing assistant Patty Vazquez (NA60500272) to reflect that in 2019 Vazquez was convicted of second-degree assault with notice of firearm allegation and enhancement.

Island County

In December 2020 the Nursing Commission charged registered nurse Patricia Anne Ainsley (RN00098894) with unprofessional conduct. Ainsley allegedly didn’t complete a required refresher course.

In December 2020 the Psychology Board entered an agreement with psychologist Aaron R. Burdge (PY60032922) that fines Burdge $3,500, requires him to pass an ethics course, and obligates him to complete continuing education in record-keeping and billing, as well as in supervision. The psychologist must submit to inspections and audits. Unlicensed interns provided services billed under Burdge’s National Provider Identifier number. Burdge kept insufficient documentation. Burdge instructed an intern to turn over client records to a different provider without assisting in the transfer.

King County

In December 2020 the Massage Therapy Program charged massage therapist Christopher W. Mason (MA60729951) with unprofessional conduct. The massage therapist allegedly improperly touched a client.

In December 2020 the Nursing Assistant Program charged registered nursing assistant Gabriel Waithaka Kimani (NA60858387) with unprofessional conduct. While driving a car with a client inside, Kimani allegedly was under the influence of alcohol and crashed the vehicle. Kimani allegedly admitted drinking beer, and underwent a blood test. Charges say Kimani was charged with failure to control speed to avoid a collision, and with first-degree negligent driving.

In December 2020 the secretary of health ended conditions on the registered nursing assistant credential of Chelsea Nicole Pierce (NA60886839).

Kitsap County

In December 2020 the secretary of health ended conditions on the substance use disorder professional and licensed independent clinical social worker credentials of William Kent Loveless (CP00004660, LW60475335).

Lewis County

In December 2020 the Emergency Medical Services and Trauma Systems Program charged emergency medical technician Derrick E. Paul (ES00110847) with unprofessional conduct. In 2019 Paul was convicted of fourth-degree assault by domestic violence with sexual motivation, disclosing intimate images, and first-degree theft.

Pacific County

In December 2020 the secretary of health ended conditions on the substance use disorder professional trainee credential of Kathryn Victoria Folds (CO60904245).

Pierce County

In December 2020 the Massage Therapist Program entered an agreement with massage therapist Denise S. Wood (MA00014484) that fines Wood $250. Wood falsely attested to having met continuing education requirements.

In December 2020 the Nursing Assistant Program charged certified nursing assistant Millicent Kemp (NC60024620) with unprofessional conduct. Charges say Kemp attempted to turn a patient by herself when the patient’s care plan required a two-person assist. The patient allegedly fell from her bed and sustained a fracture.

In December 2020 the Massage Therapist, X-Ray Technician, and Medical Assistant programs charged massage therapist, X-ray technician, and medical assistant phlebotomist Kevin Lee Vandenbosch (MA60255380, XT60415753, PC60585596) with unprofessional conduct. Vandenbosch allegedly improperly touched a massage client. In August 2020 Vandenbosch was charged with indecent liberties.

Skagit County

In December 2020 the Medical Assistant Program charged medical assistant hemodialysis technician Amy Leigh Taylor (HT60417704) with unprofessional conduct. Taylor allegedly met a former patient at a coffee shop, and exchanged text messages, photos, and phone calls with the former patient. The former patient allegedly went to Taylor’s home and workplace multiple times. Taylor and the former patient allegedly kissed, hugged, and held hands.

In December 2020 the secretary of health ended conditions on the agency-affiliated counselor credential of Greg William Fankhauser (CG60633012).

Snohomish County

In December 2020 the Naturopathy Board ended conditions on the naturopathic physician credential of Mylinh I. Vo (NT00000825).

In December 2020 the secretary of health ended conditions on the certified nursing assistant credential of Haddy Bittaye (NC60318678).

Spokane County

In December 2020 the Midwifery Program entered an agreement with midwife Elizabeth Morrill (MW00000229) that places her on probation for at least 12 months. Morrill’s license had been suspended in November 2020. In 2019 Morrill surrendered her Idaho midwifery license in lieu of discipline. She didn’t report that to the Washington State Department of Health.

Thurston County

In December 2020 the Pharmacy Commission conditionally granted a pharmacy assistant credential to Elizabeth Mae Ingersoll (VB61063131) and placed Ingersoll on probation for at least two years. In 2017 Ingersoll was convicted of criminal solicitation to possess heroin, and of reckless driving.

In November 2020 the secretary of health denied an agency-affiliated counselor credential to Dwayne Allen Kruse (CG61077431), whose Arizona licensed marriage and family therapist credential was revoked in 2015.

In December 2020 the Nursing Assistant Program charged certified nursing assistant Andrea Ann Shepard (NC10082621) with unprofessional conduct. In 2020 Shepard was convicted of second-degree theft, two counts of forgery, and third-degree theft. The convictions came in connection with theft of narcotics from a stranger’s home, and with theft of money from an elderly man who refused Shepard’s offer to serve as his caretaker.

In December 2020 the secretary of health withdrew a statement of charges against registered nursing assistant Josimar Nateras (NA60849338).

Arkansas: In December 2020 the Nursing Commission ended conditions on the registered nurse credential of Debra Marie Holmes (RN60827788).

Utah: In December 2020 the Nursing Commission conditionally granted a registered nurse credential to Gladys Marie Faleafa (RN61012955) and placed her on probation for a minimum of 12 months. The nurse must submit monthly reports on therapy mandated in connection with 2009 and 2016 domestic violence convictions.

Note to Editors: Health care providers charged with unprofessional conduct have 20 days to respond to the Department of Health in writing. The case then enters the settlement process. If no disciplinary agreement can be reached, the case will go to a hearing.
